The Health Equity Planning Table’s leads the development of a multi-year health equity strategy aimed at addressing racism and advancing equity, inclusion and diversity. The table is comprised of over 20 leaders representing organizations that serve the diverse needs of Mississauga residents and communities, in addition to four patient/caregiver advisors. Setting foundations in the OHT

Training opportunities

Measuring health equity

Ensuring equitable access to 24/7 navigation

In alignment with Ontario Health, Mississauga Health will implement its health equity strategy focusing on Indigenous, Francophone and Black communities. The health equity table was key to leading Mississauga into publicly committing to equity through the following statement:

Representing the diverse populations that call Mississauga home, we are committed to achieving an equitable system of health and wellness across each community by understanding he needs of diverse communities; reducing barriers to care; supporting the development of diverse, inclusive and anti-discriminatory organizations; and advocating on the social determinants of health.

When launched, the Home Care Modernization Planning Table will serve as a collaborative platform dedicated to strategic planning, coordination, and execution of initiatives to transition and modernize home care delivery and services.

Enhance the patient and caregiver experience through innovative methodologies.

Integrate advanced technologies that improve care integration and navigation.

Reduce reliance on hospital care for home care clients.

The Mental Health and Addictions Planning Table aims to build and execute a multi-year mental health and addictions strategy by using a population health approach to improve overall health and well-being of those with relevant conditions.

Our objectives:

Enhancing integrated care and improving timely access to care.

Promoting early intervention and holistic treatment approaches.

Improving the lives of those who would benefit from a palliative approach to care has been a focus for Mississauga Health since its inception. Partners from a broad cross-section of palliative care providers in Mississauga have joined together to support the development and implementation of a strategy aimed at advancing palliative care in our region.

This past year, we have developed an integrated model of palliative care through co-design with providers, family and caregivers, which will be implemented in the coming years. Our current focus is on advanced elements of the model that relate to modernizing the delivery of home care. Ontario Health has selected Mississauga Health as one of seven leading projects, which are currently in the design implementation stage.

Our objectives:

Advancing the integrated model of Palliative Care.

Building capacity and competency in palliative care locally.

Primary Care integration is a key determinant of success of the overall Mississauga Ontario Health Team (OHT) vision and mission. As such, it continues to be a key priority for Mississauga Health as we aim to establish a team-based primary care model, ensuring every person has simplified access to coordinated, high-quality, culturally safe and community-based health care.

The five strategic pillars of this multi-year vision are:

Providers and patients will be attached to hyper-local, team-based primary care infrastructure (Integrated Primary Care Centres – IPCC) designed to address the needs of our communities.

People without a regular care provider will have access to high-impact primary care services, with a focus on continuity of care.

Teaching, training and research will be embedded in the IPCCs to advance and modernize primary care delivery in Mississauga, recruit and retain physicians and advance best practices.

Providers will leverage technology enabling them to be seamlessly connected to acute care, specialist, community and social services to support strong system capacity, access and flow.

Development of a strong physician network to provide leadership, engagement and mentorship amongst primary care providers to support physician recruitment and retention in the region.
