The Mississauga Health Seniors Navigation Portal helps older adults continue to live well at home by connecting them to health care providers in Mississauga, before their health becomes an emergency.
A Care Navigator will help connect older adults to the services they need and offer support through the process. Services are delivered through several partner agencies in the City of Mississauga.
