On November 26, 2019, the Honourable Christine Elliott, Deputy Premier and Minister of Health announced Mississauga Health as one of the first of 24 Ontario Health Teams (OHT) in the province.
Mississauga Health is a partnership of local health care providers who are committed to better health in the community and better experiences for patients and providers. Together, we will work to make sure patients, families and caregivers have better access to the care they need, when and where they need it.
In early 2019, the Government of Ontario introduced the OHT model to integrate health care providers to work as one team, so that patients experience seamless transitions one care setting to another as well as have better access to after-hours services and community supports. Our approach is aligned with the province’s plan to build a connected health care system that is centred on patients, families and caregivers.
Our Vision
The care we provide will address physical, mental, social and emotional well-being, and will be reliable, high quality and grounded in exceptional experiences and sustainability, delivering on the Quadruple Aim.

Our partnership is anchored in a shared commitment to patient-centred health care in this community and an established track record of innovative collaboration.
Mississauga Health is guided by an Interim Governing Council made up of a diverse group of partners who are committed to creating a seamless system of care and improving population health. This leadership team are the co-signatories of both Mississauga Health submissions to the Ministry, on behalf of the community.
Over 90 diverse partners representing health and social service sectors, including but not limited to primary care, home care, community care, acute care, long-term care, social services, associations and community organizations joined Mississauga Health as members or affiliates.
