Building an Ontario Health Team for Mississauga

The Government of Ontario has set in motion a plan to build a connected health care system that is centred on patients, families and caregivers through the creation of Ontario Health Teams (OHT) as part of their broader health system transformation strategy.

The proposed Mississauga OHT (Mississauga Health) represents a partnership of local health care providers who are committed to better health in the community and better experiences for patients and providers.

Mississauga Health submitted a Readiness Self-Assessment in May 2019 for consideration to become an OHT. The Ministry received 157 Readiness Self-Assessments and Mississauga Health was one of 31 teams invited to submit a Full Application.

Our Full Application was submitted to the Ministry on October 9, 2019 but our planning will continue together beyond this date as we collaborate to improve health in the community.

Our Vision

Our vision is to improve the health of people in the community by creating an interconnected system of care from prenatal care to birth to end-of-life.

Through partnership, care will address physical, mental, and emotional well-being, and will be reliable, high-quality, grounded in exceptional experiences and sustainability.

Mississauga Ontario Health Team (Mississauga Health) announced as the first of 24 Ontario Health Teams (OHTs) in the province

The Honourable Christine Elliott, Deputy Premier and Minister of Health was in Mississauga on November 25, 2019 along with patients, families and community groups to announce the Mississauga Ontario Health Team (Mississauga Health) as the first of 24 Ontario Health Teams (OHTs) in the province.

We are on a journey to transform health care in our community and our work doesn’t end with the Full Application. Mississauga Health will continue to work with members, affiliates, stakeholders and the Ministry to advance health care in the community.

Our partnership is anchored in a shared commitment to patient-centred health care in this community and an established track record of innovative collaboration.

Mississauga Health is guided by an Interim Governing Council made up of a diverse group of partners who are committed to creating a seamless system of care and improving population health. This leadership team are the co-signatories of both Mississauga Health submissions to the Ministry, on behalf of the community.

Over 90 diverse partners representing health and social service sectors, including but not limited to primary care, home care, community care, acute care, long-term care, social services, associations and community organizations joined Mississauga Health as members or affiliates.
